Voluntary Centre Services has an exciting opportunity for a highly motivated individual to join a small team as a Social Prescribing Link Worker based in Sleaford.

The Social Prescribing Service, developed alongside the Integrated Neighbourhood Working (INW) programme provides a means of enabling primary care services to refer patients with social, emotional or practical needs to a range of local, non-clinical services.

The new team member will provide support to patients alongside our existing Social Prescribing Team in the South Lincoln Healthcare Primary Care Network (a group of GP practices in the southern part of Lincoln and the North Kesteven area).

Social Prescribing Link Workers are responsible for delivering a person-centred service, supporting people to improve their health and wellbeing by connecting with services in their community. Link Workers are a vital community resource, responsible for patient engagement and developing effective working relationships with health care professionals and third sector providers.

Annual Leave entitlement 29 days per year (pro rata)
