Voluntary Centre Services is looking to recruit Social Prescribing Link Workers to cover the Lincoln and Gainsborough areas.
Social Prescribing Link Workers are responsible for delivering a person-centred service, supporting people to improve their health and wellbeing by connecting them with services in their community. Link Workers are a vital community resource, responsible for patient engagement and developing effective working relationships with health care professionals and voluntary/community sector providers.
We have roles available to support patients alongside our existing Social Prescribing Team within the newly expanded Integrated Place Based Teams (IPBTs) which form part of the Integrated Community Mental Health Care service working alongside the Neighbourhood teams in Lincoln and Gainsborough supporting people with enduring and severe mental ill health connect to community groups, support and services.
Social prescribing can help to strengthen personal resilience and communities, and reduces health inequalities by addressing the wider determinants of health, such as isolation, debt, poor housing and physical inactivity, by increasing people’s active involvement with their local communities.
You will build trusting relationships with people at all levels, connect them to community groups and Voluntary, Community and Social Enterprise (VCSE) organisations and services as well as work with partners to provide support to organisations involved in social prescribing.
You must be a good listener, have time for people and be committed to supporting local communities to care for each other. You should have experience of working positively with people facing complex social and emotional challenges as well as have great interpersonal skills and ability to support people, community groups and local organisations.
Posts are initially offered on a twelve month contract, with the potential for extension.
Salary £23,429 per annum
Full time (35 hours per week)
Annual Leave entitlement 29 days per year (pro rata)
